Unpaid Health Insurance Can Block Your Visa Extension: The Job-Search Trap

  • People who did not work for several months because the company closed or stopped work
  • People who became regional subscribers after changing to another status such as G-1 because of an industrial accident or illness
  • People who moved dormitories, did not report the move-in, and did not get the bill
  • People whose company reported their enrollment late, so the time before workplace enrollment was counted as regional enrollment

Why Unpaid Premiums Happen

As of 2026, the regional subscriber premium for foreigners is at least 158,630원 per month. If your job search lasts 3 months, you will be billed about 47만 원 (470,000원). If you miss this money, it will show as "unpaid" at your next stay extension.

Steps — What to Check Before Your Stay Extension

  1. Check if you have unpaid premiums. Log in to the The건강보험 app with your foreigner registration number and look at "보험료 조회/납부" (check/pay premiums). If the app is hard to use, call 1577-1000 (foreign language help: 033-811-2000).
  2. Change the address for your bills. If you moved, you should report your move-in within 14 days (at a 주민센터 (community service center) or 하이코리아 (HiKorea, the online immigration portal)). Also tell the 국민건강보험공단 (National Health Insurance Service, NHIS) about your new address. If you apply for mobile bills (app or KakaoTalk), you get them no matter where you live.
  3. If you can pay, pay right away. You can pay through the app, at a bank, or at a convenience store. If you pay everything, your stay extension review is handled normally.
  4. If you cannot pay all at once, apply for installment payment. Apply by visiting an NHIS branch office, by calling 1577-1000, or through the app. The number of payments and the conditions depend on how long the premiums have been unpaid, so NHIS will explain them. If your installment payment is approved, keep the payment plan document.
  5. When you apply for your stay extension, also submit your payment confirmation or your installment approval document. If the Immigration Office tells you about unpaid premiums, explain your payment plan right there.
  6. When you start at a new company, check your 급여명세서 (pay slip) the next month to see if the company reported your workplace enrollment. If the report is late, that period stays as regional enrollment.

Things to Watch Out For

  • Unpaid premiums block not only your visa but also hospital coverage. From April 23, 2025, foreigners follow the same rules as Koreans. If you miss premium payments 3 or more times, your insurance benefits are limited. If you pay later, coverage may be recognized back to the earlier date. Check the exact rules with NHIS.
  • "The company will take care of it" does not work during the job-search period. During that time, you have no company.
  • Settle your payments before you leave Korea, too. If you leave Korea with unpaid premiums, it stays on your record for your next entry and visa review. This is even more important if you are aiming for re-entry as a sincere worker (성실근로자) or for E-7-4.
  • Not getting a bill does not mean you do not have to pay. It means the bill went to the wrong address.

Q. My job search was only one month. Why did I get a premium bill? A. Because you become a regional subscriber from the day after you leave the company. If you were a regional subscriber for even one day in a month, you may be charged the premium for that month. Check with NHIS if the amount is correct.

Q. I have unpaid premiums, but I got my re-employment extension. So is it okay? A. Re-employment permission (고용센터 (Employment Center)) and stay extension (Immigration Office) are different procedures. If unpaid premiums show up in the extension review, your extension may be limited to 6 months or less. It is safer to pay before you apply for the extension.

Q. My company took the premium out of my salary but did not pay it to NHIS. A. For workplace subscribers, the company has the duty to pay the premium. Take your 급여명세서 and report this to NHIS and to 고용노동부 (Ministry of Employment and Labor) (1350). You must leave a record showing it is not your fault.

Q. I have no money, so I cannot pay now. A. Apply for installment payment and pay the first part. A document showing you have a payment plan helps in the review. If you miss a payment during the installment plan, the approval may be cancelled, so set up automatic transfer.
